On the disability front … not all disabilities are visible . I have profound bilateral hearing loss and function okay with the advanced hearing aid systems of today. But I still get easily spooked in parking lots and walking on the streets.

...What she did not touch on was that, once you get past a 40 band, smaller cups are non existent, simply because women with large chests tend to have large breasts and there is no market for them, except for us... I am going to try an extender and see if everything stays in place.  If somebody wants to get into the men's market, we need larger bands with smaller cups.  I am a little overweight, but I am not what anybody would consider a large man.
In my search for a brassiere that fits I struggled with this issue... large band but smaller cups.  The solution I found is the one you are considering... buying a brassiere with a smaller band and using both an extender and a strap that holds the shoulder straps in place.  I did find a 44B brassiere that doesn't required an extender and bought it in three different colors.  I'm wearing the nude brassiere as I write this.  Unfortunately, the supporting frame is a bit heavy and becomes uncomfortable with long wear.  I put it on about four hours ago and will likely take it off soon.  The brassiere that has worked for me is a 42C which I can wear all day with the extender and strap to hold the shoulder straps in place.  I've bought perhaps tens of those brassieres in different colors, five of which are set aside for future wear.  Sadly the manufacturer no longer makes the 42C.  At least for now I'm not concerned but it is pretty clear to me that to get a brassiere that fits... an unpadded, underwire brassiere, I'll need to got for a narrower band and an extender.  Good luck sorting this out.
