Hi Doctors and members,

I received three different opinions from doctors, one says - no gyno , no need to anything, the other - gyno and fat = must do complete surgery, and finally = third doctor, only liposuction needed.

One doctor claimed even though I lost weight, I would still need liposuction; not sure if this is true.

Current weight = 157 lbs Height 5'5
At 132 lbs (unhealthy) = chest and stomach fat was very minimal, however breast were minimal pointy.

Ultrasounds were extremely hard to read.
Finally the mammogram I got clearly shows gyno. (Probably just minimal grade 1 gyno)

Can I get some expert responses please ?

Shall I shred fat ?
Shall I shred fat, then opt in to do gland only ?
Shall I not shred fat, and do both ?

You appear to have a small amount of excess tissue on the right side and perhaps some on the left as well.  Leave it alone?  Only you can decide.

If it bothers you, then no further diet or exercise will do the trick -- only surgery.

Direct gland excision might work -- but the surgeon should also be ready to perform some peripheral liposuction to even out the edges of the excision.

This can be one of the more difficult cases for a surgeon, for there is little room for error.  Be smart and consult an expert in gyne surgery and get it done right the first time.

Hi Doctor,

If I understand correctly, even though if I lost 10-15 pounds of my torso/chest area, I would still need to do liposuction ? 

I'm 100 % sure I want to get the glands out, but I really do not want to do liposuction.

However, if you state that it's leaves the surgeon little room for error, and since I do not want a revision surgery, neither a second surgery in the near future, I would accept to do liposuction.

I just need clarification on prior weight loss. I can shred fat quickly, but if Liposuction is required regardless; then there would be no point in losing substantial amount of fat.

If you are at a reasonable weight, there is no need to shred fat.

When I talk about liposuction, it doesn't mean that you are fat.  It is used to even out the edges of the gland removal so that the entire chest results in a smooth contour.  In fact, sometimes I use the liposuction cannula without suction -- just to even out the contours.

I think you are too caught up with the thought of "liposuction."  Realize that it is used very carefully and judiciously and appropriately when doing a case such as yours.  However, you need to consult with a gyne expert to get a best result.

I perform liposuction, Ultrasonic Assisted Liposuction and glandular excision on nearly every patient. I am prepared to use all three techniques in every case. I think quality gynecomastia surgeons should be able to perform all three even though there might be occasion cases which don't require it. Good luck.

Dr. Delgado and Jacobs have both provided very useful information based on their vast experience. As stated previously, only you can decide if it is worth it to have your gyne corrected. I would imagine so since you have been to multiple consultations and are also on this forum. Assuming you want to have surgery, just be sure you are choosing a doctor who is truly experienced with male breast reduction surgery. Gland removal is a must and perhaps a small amount of lipo contouring to further define the chest. It won't add but a little bit of extra time and really no change in your recovery. Think of it as the cherry on top of your sundae.
