hey docs im at the 4 1/2 month post op mark and im starting to get a little worried about my left side. it seems to me like theres still a little bit too much gland  wich is causing the puffiness and when i flex the lower part of my nipple sinks in a little bit, it doesn't bother me that much its more the puffiness that bugs me. My doc said it would smoothen out but i can still feel some hard tissue on the top portion of my nipple. Just wondering what you guys think from looking at the pics?

Thanks!

Tough to answer your question because we have no "before" photos to compare.

Remember, each side heals at its own rate.  Further, scar tissue may develop asymmetrically as well.  Time is on your side.  Massage can sometimes help.

Be patient and do return to your surgeon to discuss.

I agree with Dr. Jacobs.  Swelling will resolve at different speeds from side to side, and if the situation was not identical pre-op, that can also account for asymmetric healing.  Swelling is still present at this point, and I encourage patients to massage to help with this.  Give it more time, but see your surgeon as well, so that if something is required, he or she can guide you accordingly.
