Hello I'm a 17 year old male.

I'm 5'7 and 130 lbs.

I've had puffy nipples for as long as I can remember. There is no hard tissues or lumps under my nipples. I will attach pictures to the post.

When I'm cold it becomes smaller, but it never stays like that.

Do I need to undergo surgery to fix this or can a healthy diet and workout get rid of this?

If I do have gyno is there a way to get rid of it without surgery?

Very  mild puffy nipples -- but the puffiness is basically breast tissue directly beneath the areolas.  Since you have had it for many years, you have two alternatives:  live with it and mask it when desired with a snug compression garment, or have surgery to very carefully remove the excess while making sure you do not have a post op crater deformity.  Best to go to an experienced gyne surgeon for this type of work.
