In doubt about choosing between 2 surgeons, need advice

Hello, I have recently discovered I have gynecomastia (big areolas + gland tissue).
I went to two different surgeons here in Brazil and one recommended removing the gland tissue and reducing the areolas manually, the other said the areolas would be reduced naturally with the tissue removal.  
Do large areolas diminish with the tissue removal solely?
My current situation is exactly like the before pics in this case imgur.com/GGWb0bV.jpg
He had really large areolas, but they were reduced to a great size. Would liposuction and breast tissue excision solve the problem or did this guy get his nipple manually reshaped?
I'd love to talk to someone with some experience regarding the issue.

When adequate gyne surgery is performed, the areolar diameter will always diminish.  It is like looking at a circle on a blown up balloon.  Let the air out of the balloon and the circle will shrink -- automatically and quickly.

So, too, the areolar diameter will diminish right in front of my eyes on the operating table.  And all this will happen with minimal scars.

Some doctors will perform areolar reduction surgery.  This frequently leads to a widened scar around the areola and the areola frequently stretches out again over time.  In rare circumstances, there can be diminished blood supply to the nipple which could cause problems in healing or in severe cases, loss of the nipple itself.

I would agree with Dr Jacobs, its interesting that I use the a similar analogy with my patients. You can always perform a reduction ,which is rarely required,as a secondary procedure.
