In December of 2017, I had gynecomastia surgery. As I told the doctor, I worked out and was generally happy with my chest except for my puffy nipples and perhaps some excess tissue behind the nipple. Echoing what others have posted here, I said I wanted my chest to look like it does when it's cold.

I am post surgery five months and incredibly disappointed with the results. Not only was there a significant amount of tissue lippo'd from all over my chest, drastically reducing my over-all "good" mass, but the two breasts are completely and very noticeably, different sizes. On one side, all of the chest tissue below my left nipple was removed, making my chest look flat and deflated. And no exaggeration, looking disfigured. The other side, more tissue remains which looks more normal. But my chest as a whole, is completely unbalanced. And making matters worse, he did nothing to address my primary issue, the puffy nipples. Which now look worse because my chest isn't nearly as broad.

I am hopeful that the puffy nipple issue can be addressed with a follow up surgery. Even though my plastic surgeon has offered to do it for free, I will be doing what I should have done in the first place, working with gynecomastia specialist.

But my question is this: Is it possible to put tissue back into my chest to produce and more balanced and more athletic look like I used to have?

It sounds as iff you really didn't have full communication with your surgeon.

Part of gynecomastia surgery is to reveal the muscle underneath, which necessitates fat removal.  If you wanted that fat left intact (leaving your "good" mass), then you should have made that clear to the surgeon.  In any event, it is generally not possible to put back tissue on a gynecomastia patient, although some enthusiastic surgeons might recommend extensive fat grafting.

Treatment of puffy nipples requires removal of the gland tissue just underneath the areolas.  IF that was not done, then you should consult an expert for any revision surgery.

Chest contouring in men can include the removal of fat and / or glandular tissue as well as adding fat in order to enhance the overall shape. Good luck.
