Hello!

I need some help, i have low self-esteem cause my breasts.
My question is:  Is this gynecomastia or i'm just fat?
Should i ask for help?

Yes, you do appear to a a mild case of gynecomastia.  You are relatively slim, so losing weight probably would not help you.

You have two choices:  live with it or seek an expert opinion for surgery.

I don't have any other symptoms (swelling, tenderness, pain etc.)
Is it possible to have gynecomastia without any symptoms?

You do appear to have mild gynecomastia.  Typically, gynecomastia that arises during puberty may have no symptoms, other than progressive appearance of puffy nipples, lower chest fullness, or some combination of both. When it occurs at later points in life related to medication or steroid use, there may be symptoms such as pain or sensitivity. It's not clear if diet and exercise would give you improvement based on the photos and history supplied.

I'm 25 years old, I never used steroids and medications.. and yeah, i have puffy nipples too,
I read about the "pinch test" from Dr. Elliot Jacobs. He said the pinch should be the same all the way down. But around my nipples it's not the same so.. so i think i have gyne for sure.

I agree with the others. There is probably a central area of glandular tissue that will not go away without surgery. Fortunately the surgery is very successful with a pretty easy recovery. No need to always having to worry or think about how your chest looks. Go get a consult from an experienced gyno surgeon. Good luck.
