If you have puffy nipples and are having surgery, then IMHO, the result immediately after surgery should be smaller diameter areolas which lie flat and flush with the surrounding skin -- all the time.

Sir i had surgery 4 weeks ago but my nipples are still little bit puffy when i see myself in the mirror it looks normal. But when i roll my hand over my chest i can feel them differently. I told this to my surgeon but he said that my nipples are normal. When i press them i feel no gland beneath them. How can i make sure if my nipples are normal.

They don't look particularly puffy on the photo. it might just be some residual swelling as you are only four weeks after surgery. Hang in there.

I would agree that your nipples look pretty flat in the photo.  I always tell my patients not to worry if there is some prominence of the nipples when they look down from above--that's normal anyway.  At four weeks some puffiness is common as well.
