No, working out will not improve the nipple.

At four months post op, it would be best to consult with your surgeon to see if HE THINKS you may have residual tissue or perhaps some scar tissue.

I work with many Bodybuilders with Gynecomastia. Most complain that as they build up muscle, what sits on top of the muscle just gets pushed out further.  If the contour problem is from loose skin, building muscle can fill a loose skin envelope to a degree.

If fat is the major component, working out can help reduce overall body fat.  However, you cannot target where the fat comes from. 

Working out does stimulate thin muscle under the skin of the aroeola that can flatten puffy nipples, but it is not practical to keep stimulating this muscle. I have seen patients with breast pain from such constant stimulation. 

If the remaining deformity is gland, then Revision Gynecomastia Surgery may be an option.

The cause of your continued puffiness may be either residual tissue under the nipple/areola (usually glandular but may be fat) or persistant swelling. The simplest to thing to treat at this point is persistant swelling. To treat this I routinely perform a kenalog 40 steroid injection. It is quick with little risk (if performed correctly). I have seen this work remarkably well when there is persistant swelling. Revisional surgery can always be considered if this does not work. Always try the simplest solution first!
