I had gynecomastia surgery about 45days ago. This "scab" still remains. The other side has healed. It's only connected by this yellow/white substance. Is this normal and what is this under the scab?

Sorry to tell you this but this appears to be a dead nipple -- I cannot tell you why.  Eventually the "scab" will fall off once the tissue underneath has healed.

Be sure to return to your surgeon to discuss this and inquire about possible scenarios going into the future.

Be sure to post more photos once the "scab" has fallen off.

If that is what I think it is, you have complete necrosis of the nipple and areola. You need to discuss with your surgeon.
