Hi,
It's been over 2 years now since I had surgery. The thing that concerns me is that I seem to have a ridge of scar tissue in the upper pectoral area; it makes it look almost like my pec muscle is missing or hollow, and it a defined contour when viewed from the side.
Will this go in even more time or should I be looking into having something done about it? If the latter, what are my options?

You have a mild crater deformity. The ridge is at the transition where the breast tissue was removed and the normal fat of the chest remains. In general this type of irregularity can be avoided and/or treated with liposuction. If the other skin over the respected area is too thin, fat grafting could be done.

Is there any chance it would go with weight loss or certain exercises to build up the lower pec?

Agree that your irregularity is related to the underlying irregularity of fat and residual tissue with the resultant crater deformity.  Most likely, some fat grafting and re-distribution will aid in getting you a smoother contour.  I don't think chest training or diet alone will resolve your concerns.

I agree with my colleagues -- no amount of weight training will help.  You have craters on both sides and in my experience, the best, most reliable and immediate improvement can be secured with fat flaps.

You will need to seek the opinion of an expert in gyne surgery for this.
