How old do I have to be to get the surgery done?
I am 16, am I old enough? On this websites home page, it said I had to be 18

You can get the operation done at any age, but PS's 'prefer' you to be past puberty and the raging hormones...

If you're under 18 you will need your parent's consent too I believe.
