3 and a half month post surgery, it's slowly getting worse

when I first took off the bandages I was happy. but now it seems to be worsening, almost coming back to how it was pre-op. my nipples are getting very "pointy". when I flex my pecs I can feel a very tender, pointy spot that sits ungracefully in front of the muscle.



there don't seem to be much fat left in the area, just a very tender spot, it feels like the skin is getting loose. I am also losing weight at a natural rate, it might have to do with it.

diagnosis? options?
