Help please

I am 16 and I have had puffy nipples since i was about 11/12. My right nipple has always been much bigger than my left one. I dont have a lump under them and it feels like I have man boobs but I have puff nipples aswell. Im not sure if this is still puberty but Ive had it for a long time since a young age.

Even if you can't feel the breast tissue, it is most likely the cause of your puffy nipple. Especially, in younger patients who are lean.  The breast tissue effectively bulges out, which causes the nipple to appear puffy.
