How to start booking surgery

Ok well I was wonder how you would go about booking the surgery like where to start. For example like do you need to get a forum or anything from a doctor. and whats steps do I have to take before setting it all up. Next I was wonder if Dr. Fielding is the best choice cause I have read many good things. I am in pretty good shape with puffy nipples and just need some guidance here. What should I expect to be paying and what is covered by OHIP and whatnot. Plus to get this coverage is there any other steps I must take or do I just show my health card??? pls help me, btw I am 19 and I doubt it will change now..large or smaller I mean, I have not used any forum of drugs to try an cure it so do you think I should try that first? and I have had it since like 13-14...
