insurance coverage lipo vs gland removal

is insurance any more likely to cover gland removal vs. lipo (fat removal)?

just exactly how is gland removed?  it's pretty clear how fat is removed (lipo).  do they inject it with something to break it up/loosen it up and then suck it out?  just curious as in a surgeon doesn't have a large opening to work with.

The make an incision around the bottom of the nipple,  and cut the gland out. The should also do lips around the nipple to level things out after the gland is removed.

so they can get to the gland tissue wherever it is with a small incision?  my gland is pretty spead out.  it is true the bulk of it is under if very near the nipple/areola area.  but to get it all, you'd have to go a good inch of two in every direction (from the nipple).
