Dr. Bermant,

Post-operatively speaking (weeks/months),... have you ever injected Kenalog (or any other coritcosteroid) into a patient's areola if it happened to be excessively inflammed, protruding, indurated, & painful?

Can the steroid injection eventually cause a depression deformity; especially at the injection site?

Generally, how soon is 'too soon' to inject the steroid into excessively inflammed tissue or scar tissue?

Kenalog can be a powerful tool for scar care.  When a scar is becoming hypertrophic or thickened, a steroid injection can help slow or reverse the process.  Steroid injections can interfere with healing and also result in thinning of tissue and depressions.  Each case needs to be individually evaluated by the doctor familar with the problem.  It is very rare in my practice to need steroid injections for the scars.
