Nipple appearance and surgury

got the surgery from a "top rated" surgeon, it was $6,000. 
there is a slight improvement, but the nipples stick out, even more than before the surgery. 
the doctor said that anything to do with fixing the nipple is not included in "surgery for gyneclomstia"
the doctor offered to "shave the nipples off" in an additional operation for $500. 
any thoughts on this? if I were to go to another surgeon in regards to this, would the quoted price be much higher than $500?
