I had a really nice chest until I gained 45 lbs a few years ago.  Two years ago I decided to lose all the weight and I did except in my chest area.  I tried lifting weights, dieting, running, supplements, etc.  and nothing seemed to work.  I have good definition in my abs, but I continue to have fat in my chest.

The fat is not behind my nipples, it's mainly in the axilla and lower pectoral areas.

I went to three different plastic surgeons (one whose listed on this site) and they all told me that I had glandular tissue that needed to be cut out and that's why it was not reacting well to diet and execise.

I then went to a breast cancer surgeon who gave me an exam and then ran both a mammogram and an ultrasound.  The showed me the results and there was no glandular tissue!  It was all fat!  

So how do I approach a plastic surgeon now with these results?  They all seem to want to do areola cuts in their procedures.  This breast cancer surgeon said that they should be able to do the entire procedure with liposuction only.

Are there any doctors who specialize in pseudogyno over regular gyno?

In most cases, there is both fatty and glandular tissue. Even highly skilled doctors are sometimes surprised at what they find in the course of surgery and they are not always sure of what is there until the surgery is done.  I think it would be best to find a surgeon who will keep his options open and be flexible enough to adapt to what is found as the surgery progresses.

Then too, I am one person who does not like the distinction between what some refer to as True vs Pseudo Gynecomastia.  The Emotional and psychological effects are the same in either case and in most cases, there is no physical pathology.

The breast cancer surgeon ran a mammogram and an ultrasound and developed all sorts of images of the tissue in my chest.  She said that it was all fat and no glandular tissue.  

She showed other people's images with glandular tissue and pointed out the differences between the two.

Most of my fat is in the axilla area near my armpits and not over the actual pectoral muscle.  It makes it look loose and saggy, but not like boobs.  If I go to the gyno photos I cannot find anyone with a chest like mine, they all have protuding nipples and breastlike pecs.  I don't have that problem, but I have fat in the axillary area near my armpits.

They did find an artery in my chest (only on the right side) when they ran the ultrasound.

I also had my estrogen levels check and they came back 10 which is on the lower end of the spectrum.  I don't have an estrogen problem, mine were caused exclusively by a weight gain.

As far as choosing a surgeon for treatment of gynaecomastia (irrespective of it being pseudo or 'true' gynaecomastia) consider the one you feel is best able to 'sculpt' your chest to a shape you'd be happy with.

Explain to this surgeon what the cancer specialist found and explained to you.  Say that you would be happier to proceed with surgery if your chosen surgeon would speak with the cancer specialist to discuss your case.  Any plastic surgeon worth his salt would be happy to learn as much as possible about individual patients to improve their own knowledge this way.

It's my guess that an oncologist or cancer specialist is going to know a great deal more about different bodily tissues than a plastic surgeon and a plastic surgeon will know a vast deal more about physical aesthetics and tissue manipulation than an oncologist!  Get their heads together and you will find the way forward!

Well, I just had a mammogram about an hour ago.  This doctor, too, said it was almost purely fat.  However, I can FEEL the glands, and I know it's the glands that are causing the protrusion.  He told me that sometimes, mammograms won't show all of the glandular tissue, so keep that in mind.
