So my nipples do appear puffy and i can tell there is definitely tissue/fat i can grab underneath the skin, however, there is nothing directly under the nipple at all. Does there have to be overdeveloped breast tissue for the nipples to be puffy or can this be caused by excess fat?

It would be helpful if you could post some photos.

Usually, puffy nipples are the result of excess breast tissue directly beneath the nipple and then the nipple is pushed out.  Feeling for "hard" or "firm" tissue is a fool's errand -- breast tissue can feel soft and fat tissue can feel hard.  What makes the diagnosis is simply the contour.
