spironolactone is a diuretic (water tablet), used to reduce body water. clinical situations were its used is heart failue and ascites ( fluid build up in the abdominal cavity in chronic liver disease). it is essentially an aldosterone receptor blocker in the kidneys, the end result of this function is to lose Na+ and hence water. one of its well recognised side effects is gynecomastia, but the good news is that this is reversible once the drug is stopped. From my experience usually the patient will complain of breast pain and tenderness as an early sign and this is an indication to stop the drug if possible.
Hope this will help.
