Just curious what the difference is between gynecomastia and puffy nips?

Is puffy nips only gland related and gynecomastia is gland with fat, or just mostly fat?

Is one treatment better for one or the other?

The definition which I prefer is that of any enlargement or feminization of the breast.  This would include Puffy Nipples, Fatty breasts in the obese, or Fully Developed Breasts.

Since the effects are primarily of an emotional nature, the cause and precise nature of breast development is inconsequential;  The result is the same.

Most gynecomastia is physically harmless, But it can be a symptom of serious problems and I think is is generally wise to see an Endocrinologist just to be safe.

To continue with what Paa said:

There can be several symptoms under the umbrella of "gyne".  Yes, puffy nips is one of them.

I'd say ask your PS, but it's my understanding that, in most cases of puffy nips/areolas, the culprit is mostly glandular tissue with small pockets of fat.

When you say puffy nips, do you really mean puffy areolas?

As far as treatment.  Excision is common for gland removal behind the areola (usually a small incision around the lower half of your areola) and liposuction is used to remove the fatty tissue (a cannula is inserted usually under the arm pit area and it sucks out the fat).
