What Age To Consider Gynecomastia Surgery is a common question.
There is no one age for when tissues might resolve on its own. I have seen older men who had gland swelling after stimulation by some hormonal event. Once the stimulation was removed, the swelling went down to the point that there was no contour deformity. Once the gland is out of the swelling phase (fibrotic gynecomastia), it will not go away no mater how long. Gland there 2 years after stabilization just does not go away. For my patients, I recommend stabilizing the problem at least a few months after there have been no further gland growth or symptoms before considering surgery. So it is not the age, but timing after stabilization that counts.

For most teens who develop gyne during puberty, there is a good chance that it will diminish in size if not disappear entirely.  However, for those whose gyne remains unchanged for two years or more, then the likelihood that it will disappear spontaneously diminishes significantly.  There is obviously no physical harm in waiting.  Since this is a benign condition, there will be harm except for the possible psycho-social side effects.  Age is not the determining factor.

The rule of thumb is that gyne which has not disappeared and is stable for two years or more is unlikely to disappear and then the only recourse would be to wear a snug body compression garment or to have surgery.
