Please help.
can i reduce this by exercise? or do I need surgical procedure?

Looks like gyne to me bro.
But for a confirmed diagnosis you will have to visit a qualified doctor who will examine you and confirm your diagnosis. If it is confirmed as gyne your best option to get rid of it is surgery. Exercise and fat loss does not in any way help get rid of the glandular tissue, surgical excision is required for that. However, if its pseudo-gyne i.e. just fat, exercising is the safest and healthiest way to get rid of them.

You are reasonably slender and still show some evidence of gyne. While dieting and working out are great, don't count on them making all the chest excess disappear.  You would probably require surgery to get it absolutely flat.

Best to consult with an experienced gyne surgeon in your area for an opinion.

You do not need a referral from your family doc -- go directly to a plastic surgeon.  Best to do some research and find a skilled and experienced gyne surgeon in your area -- or consider traveling to see one.
