From what I've gathered there is some, but not much, evidence that melatonin supplementation could be associated with gynecomastia. I didn't find much in the literature other than a case report of an ALS patient whose gynecomastia resolved after melatonin cessation.

I had a good surgical result. I also get a sleep benefit from taking melatonin, but certainly don't want to risk recurrence.

I'm trying to balance what seems like a fairly minor risk against a moderate benefit. Any recommendations?

Melatonin is not on my short list of supplements to worry about.
