I had idiopathic gynecomastia. I had surgery just over 6 months ago where both breast tissue and liposuction was removed through an armpit incision. The area around the nipple became very bruised, scabbed, then red. I compressed the area of for 8 weeks, and have been applying silicone gel since however one nipple has ended up very clearly more red and has more scar tissue under it than the other. Would this be a case where steroid injections would be appropriate to attempt to break down the scar tissue and reduce the redness of the region, or is it a matter of patience and waiting for the area to heal naturally?

You have significant areolar loss and hypertrophic scarring.  This is not an easy fix. You should see an experienced surgeon.

It looks like hypertrophic scarring on the surface. You will need some topical treatment and might also benefit from IPL. You need a good exam.
