What determines the cost of an operation. Is it just a flat fee or do you have to pay more if you have alot of fat to extract? ???

Probably the more work/time needed to do the procedure, you will pay more... Time is money!

some people could break it down for you from their bills.  think surgeon's fee might run 2-3k US or so, but then you add in hospital stays, anesthetic, and everything else, and it adds up fast.
