3 months post surgery - left nipple puffy :/

I had my surgery exactly 3 months ago and my right side looks almost perfect (a little scar tissue, but it makes it look like muscle - no puffiness) but my left side looks even worse then before surgery For the past 2 months I've been massaging left side about twice more then right side and it doesn't seem to work. (rough massage 40min to 2hours per day)

I am sure that surgeon removed everything as my chest was completely flat after surgery - What are the chances gyno came back so soon ? If not When should I start thinking about cortisol injections?
