I am 4 months post op and my nipples are still puffy and sensitive. Sometimes they are hard and very tender and sometimes they aren't. I did massage myself to try and break down the scar tissue and right after i finished the massage it seemed that the scar tissue was getting smaller and smaller, but the next morning the scar tissue seemed to do nothing, and my nipples seem to still be swollen. I thought my nipples would be flat by now but they arent and i know there is a substantial amount of scar tissue and my question is should i keep massaging, or am i elongating the healing process by massaging?

Massage can be helpful in breaking down swelling which can lead to scar tissue.  But sometimes massage which is too vigorous is counter-productive -- it may induce even more swelling.

In your case, would suggest you return to your surgeon for a followup evaluation -- perhaps an injection of kenalog might be helpful at this point.
