Having seen a lot of questions about what people can expect in different provinces (and being quite different from healthcare in the US) why don't we try to gather this information in one thread.

I propose a poll:

1.   What province/territory do you live in?
2.   What is the name of your provincial health care system?
3.   What surgery did you have (excision, lipo, etc)?
4.   When (year)?
5a.  Which part(s) was covered and which part(s) did you have to pay for?
5b.  If you had to pay how much was it ($CAN)?
6.   Where was surgery performed (clinic, hospital, etc)?
