I have the option of choosing between the standard Aetna plan or (slightly more costly) Blue Cross Blue Shield Delaware. 

1) Anyone know if one is better than the other about reimbursement?  Is it better to ask ahead the insurance company ahead of time or just submit it afterwards with proof of glandular tissue?

2) If I decided to get a little lipo on the abs and pay out of pocket, will it make it more likely that insurance will deny the claim?  I don't have that much belly fat, but the surgeon I consulted said he could do it while I am under anesthesia for an extra $1500.

Can't answer about the differences between the two insurance companies.  Sometimes it is better to submit for pre-certification -- and then, sometimes not.  If you have an insurance broker, this is one question you could pose to him.

As for lipo of the abdomen, this is purely cosmetic and would ordinarily not even appear on an insurance claim.
