After the surgery is over, I understand that it is mandatory that someone pick you up. Does this person have to physically come in to the office and like sign you out or something? I would like to keep the procedure a secret. What is an excuse that can be used, so that the person picking me up doesn't find out?

That person picking you up doesn't need to know what you had done.  Just be sure to alert the OR staff and your plastic surgeon that you want it kept confidential.  They'll wheel you out in a wheelchair and help you into the car.  But everyone should stay mum about what you had done.  No problem.
