My nipples appear very pointy and pretty puffy at times, also feels like there is a small pea sized ball if i squeeze behind the nipple. This makes wearing light shirts difficult, wondering if surgery would resolve this?
I also work out a fair amount and have pretty low body fat.

You appear to have the slightest amount of excess tissue immediately below the nipple (seen best on the lateral photo).  I have operated for this on a few guys so that the nipple would lie flat and flush with the skin.  Believe it or not, the operation is very tricky because too much removed will result in a crater deformity.

However, be aware this is minimal gyne and most guys would give anything to have a chest like yours.

Hi,

Thank you for the input. How likely would you say it is that the surgery could go wrong?

I appreciate that compared to many men this is nothing, but that doesnt help me as this is something bothers me quite a bit and if I can get something done about it I will

This procedure may appear quite simple -- it is not.  There is very little margin for error -- between not removing enough and removing too much.

If you wish to explore surgery as an option, would strongly recommend you consult with a gyne expert in your area -- or consider traveling for such expertise.
