Hello. I've had gyne since puberty. I did a lipo procedure 4 years ago and didnt get hardly any results. I am considering another one here soon. i found a surgeon that I like. He is hinting to me to do the double incision mastectomy along with a free nipple graft for better and flatter results but I dont feel its that necessary. He says we can do the traditional gland removal along with lipo but i might have skin left over. I've been following specialist that download videos and pics all day long with guys with the same size chest like me and have seen good results with the standard gyne surgery. here are some pics. Which surgery should i choose?

Although you appear to be overweight and would do well with weight loss, I would absolutely NOT do a double incision operation.  Aggressive lipo with possible gland excision by a gyne specialist could probably provide you with a nice looking chest, although your skin might be slightly lax due to the original overweight condition.

Best recommendation is to seek another opinion from a gyne specialist iin your area or consider traveling for such expertise.

Thanks Dr.Jacobs. I've been to about twenty to thirty consultation both in person and virtual, and most of them recommend what you recommended. The specialist I know are over my budget and cant afford them. The doctor that I've narrowed it down to is in mexico and is a specialist on this site too. I feel comfortable around him and I think i'm just going to have to take a chance of just doing the lipo and removal of the tissue. Hoping for the best!!

I understand your thought processes and decision, but one word of caution.  You had gyne surgery 4 years ago -- and the scar tissue resulting from that operation is still there, distributed under the skin across your chest.  It will never disappear.  Before you consider another operation (and "hope for the best"), realize that revision surgery is a different animal and you really need a top-notch specialist in order to get the best result.

First I would note that from your previous surgery optimal results require removal of glandular tissue and fat from the breast. I am not certain what you mean by a "double incision mastectomy." In any case you certainly do not require an inframamary crease incision or a traditional mastectomy incision which would typically include removal of the nipple. After one failed attempt at correction perhaps you should consider someone who specializes in the procedure.
