Do you just tell a surgeon that you want the surgery and you want it covered by insurance? Will they do whatever they can to get it covered?

Insurance coverage for gyne surgery varies significantly with each company.

If you think you might be covered, then you should check with your insurance company and request their criteria for approving coverage for the surgery.  Then make sure that you fulfill each of their criteria.

The surgeon may then submit a letter requesting pre-approval for the procedure.  And then, even if approval is granted, insurance companies will sometimes deny coverage.  Unfortunately, insurance coverage is a crap shoot.
