what happens if a skinny guy gains weight (fat) after gynecomastiasurgery with liposuction...
will the rest of the body get fatter and the chest will have indentations where the lipo was performed?

In the ideal world, every patient is at his (or her) ideal body weight prior to undergoing surgical procedures, and they maintain that weight afterwards.  In reality, that doesn't always happen.  Patients who undergo gynecomastia treatment are no different, and depending on the nature of their gynecomastia, they may find that weight gain does result in increased fat in the chest area.  This may be particularly true of patients who are skinny when they have surgery, since the lipo-contouring that is done is aimed at making the chest look good at the weight the day of the surgery.  In many gynecomastia patients, part of the problem they suffer from is that the fat cells in the chest area grow more when weight is up, and they give up that fat--and shrink slower--during periods of dieting.  Contouring the area with liposuction reduces the number of these extra sensitive fat cells, but some are left behind.  If only a modest amount of liposuction is required during a procedure in a "skinny" guy, and then he goes on to get fat, those cells may fill more than cells elsewhere, resulting in a fatty chest.  If a subareolar resection of tissue was carried out as well, this area may seem a little more hollow after weight gain. 

So the answer to your question is "possibly."

It is likely your chest will be indented but it will be more difficult to gain weight in the chest area.  The reason is, that your fat deposits in that area will have been removed.
