Will losing 20-30 pounds before the op help the final outcome? I have a fairly severe case and a bmi of 28. Im losing weight pre-op to reduce the neccesary skin removal and other complications.

Interesting question. I have asked a number of my colleagues this question have gotten many different responses. My personal approach is to have the patients at or near the weight they plan to be  long term. In others words to loose  forty pounds for surgery and then put the weight back on after I do not find helpful.

Losing 5 lbs or so prior to surgery does not do anything.  But losing 20 lbs or more can have an effect and if it is possible, I would recommend losing the weight prior to your surgery.

Most guys are so happy after surgery, that they do not put weight back on -- but I have had a few exceptions.
