I had surgery 7 months ago which was incision only NO lipo,my nipples are still puffy and when I went to ask my surgeon about it he didnt understand why they still were slightly puffy. He felt around the nipple area and there was none to minimal gland left to stop sunken nipples. When I feel the area I feel no evidence of a big portion of gland also when I flex my chest seems flatter,but once I stop flexing my nipple area is back to poking out. My questions to the Docs is do you think by not doing the lipo that it will cause an empty space in that area were the gland was and just stay that way? What are your opinions on this matter. Thanks for your time.

I don't really ever do a gynecomastia procedure without some liposuction, even in leaner individuals, since you get some re-draping of the skin and some skin tightening, as well as the benefit of contouring.  Without that, it's possible that the skin "memory" of the nipple and areola remains, although the tissue behind the nipple has been removed, causing it to remain "puffy" or prominent looking after the procedure.  Not sure if that's the situation you're in.  Photos might be helpful.

Absolutely not. It sounds like you doc did a good job.  Many gynecomastia surgeons tend to lkeave tissue under the niopple to prevent the skin from collasping, this is a goio way to go.  I amn more agressive and in do have patient that complain about the collapse of the nipple area.  Having the nipple alittle outward is very normal.

But the thing is that my chest still protrudes out making me unable to wear white T-shirts or lighter shirts because they still stick out.When I pinch or flex there seems to not be much gland there,but when I stand at a regular stance from the side my chest pokes out.But it seems that without the lipo like the Dr said above,could it be possible that its more of a contour problem? Just feels like fatty skin slouching.
