I had surgery 18th August.

Gland and Lipo performed but I am worried that gland had been left as my chest is still very similar.

I have hard lumps from my Lipo incisions to my nipples and puffy nipples.

I am not happy with the result, am I panicking or is this cause for concern and possible revision?

Please see picture attached, top one before, bottom one 5 weeks after.

While you are still in the early healing phase, I think you should be looking somewhat better at 5 weeks than what appears in your "after" photo.  There is really nothing to do now other than to be patient and wear your compression garment.  Eventually, at 4-6 months post op, if it still looks the same, you should speak to your surgeon about the results and what options are available.

Thank you for your reply Dr. Jacobs.

Is was very flat after surgery and looked very nice. Do you think it's likely left over gland or fat left over?

My chest is fairly built up but this can grab the a hand full, but doesn't feel like gland just extremely puffy.
Could it just be swelling or is it beyond this stage now?

At this point, it can be swelling, early scar tissue, residual gland or any combination.  That is why patience is absolutely needed.  At 4-6 months, you will have a better idea of the results of the surgery -- and that is the right time to discuss the results with your surgeon.

Looking at 2 days post op can you give a clear opinion on my case?

The one day looked great.  At five weeks, you are obviously swollen -- it is much too early for scar tissue to account for what you have.  And gland doesn't re-grow like this.

Have you been wearing a strong compression garment?  It would help.

Yes, I've been wearing the vest religiously for 5 weeks, 24 hrs. Work, gym, sleep.

So it is possible the gland hasn't been removed at all? I can't feel the same as before the surgery, it is now puffy and hollow beneath the nipple.
