Hello everyone,

I am a 35 year old male who has has issues with the pectoral and abdominal area of my body. As far back as I can remember I have had somewhat of a sagging chest. No matter how many push ups I do or hard I work in the gym on my chest, the end result is the same. It seems that part of my chest has some definition but the rest of it sags. As for me ab section, its the same story.

I would like some medical advice (if possible) to see if I have gynecomastia and if I am a candidate for possible surgery. This is something that has caused esteem issues in my adolescent and young adult years and now I would hope that there is something else that I can do about it besides diet and exercise.

The photos are not sufficient to tell us a complete story.  But you do appear to have excess tissue on your chest, some lax skin and puffy nipples.  Those alone would require appropriate surgery.

Suggest you do your homework and find an experienced gyne surgeon in your area -- or consider traveling for an expert.

The pictures are oblique and one is in the middle of a muscular contraction. The pointy shape around the nipple - areolar complex looks like gynecomastia. As far as skin sag gynecomastia surgery is not a correction for decreased skin elasticity.

Hello Dr. Jacobs and Dr. Pensler,

Thank you both for responding to my original post. I can't reply to your responses with extra pictures because the system keeps saying that the pictures I was trying to post are too large. It was mentioned before that the previous pictures were not sufficient enough. Secondly, in on one of the previous pictures I am not flexing any pectoral muscles or in doing any muscular contractions. What I was doing was showing how much of the breast moves and there is no definition in the lower region of my chest. Hopefully I can post new pictures that will give a better view of the issue.

Thanks for the better photos.  They confirm my initial impression:  you have gynecomastia, puffy nipples and some skin laxity due to being overweight.

No amount of weight loss will make your gyne disappear entirely.  And having been heavy has contributed to some skin laxity, which will affect what any surgeon can do.

Again, suggest you visit an experienced gyne surgeon for an opinion on what might be possible for you.

I can't see your original photos. but the ones you've posted more recently clearly show gynecomastia.  I join Drs. Jacobs and Pensler in suggesting a consultation with someone who deals with gynecomastia on a regular basis, as your situation presents a number of challenges that would be best handled by a gynecomastia specialist.
