my nipples have always been like this for as long as I can remember. in the pictures below, my nipples were hard due to the cold tempature, but are normally puffy. if this is gyno, how bad is it and how much would surgery be? thanks!

You appear to have gynecomastia. Typically the nipple areolar area increases in size during puberty with a  mass that can be felt under the areola.In most cases the mass regresses significantly with time but in cases with tissue the size of yours the likelihood of regression is very small.

I would agree--that's gynecomastia, and it looks like it's mostly glandular tissue.

You have classic puffy nipples -- and most likely some additional excess tissue.  Assuming this has been present for years, it will not spontaneously disappear.  The only way to permanently remove the tissue is with surgery.

Do your homework and find an experienced gyne surgeon in your area -- or, consider traveling for the right surgeon.

Yes, pretty classic gynecomastia.  The good news is it can be surgically corrected.  Be sure to find a board certified plastic surgeon with experience in gyne surgery.  You'll most likely need liposuction as well as tissue excision.
