Hello,

I had a "mild" case of gyno, got them glands cut out (excision only since I am very lean and muscular) where the doc used nearby fat (fat graft) and sewed it to the nipple not to create a crater.
Developed a hematoma the evening after surgery, had to come in again and the doc had to re-open the wound to drain the blood. That was fun.

He said there is no need for a compression vest in my case, since there was no lipo done and there isn't any excess skin really. He also said I'd be good to go train my chest after 2 weeks. Apparently he seems to see my case as a very simple case and I shouldn't worry. I kind of believe him since I never really even had any pain so far during recovery. However I still got myself a vest that I wore the first ~5 days after I read a lot from gyno specialists (Jacobs, Silvermann etc.) online.

Now today when I woke up both my nipples were puffier than before and I felt numerous very hard knots in the area around and under the nipple. Could this already be scar tissue, only 10 days after surgery? Since my doc doesn't care if I use the compression vest I don't know what to do, could compression help getting rid of scar tissue faster/make it that no more scar tissue will be formed, or would it have quite the opposite effect since blood flow would be restricted?  Would it be too early to start massaging?

Also, my left nipple has a crease/fold if I flex. Tethering? Would compression not make this even worse? Massage?

It is still quite early after surgery -- there are a lot of things going on.  And you seem to have a lot of issues as well.  It might be helpful if you could post some photos for us.

As for a compression vest, I cannot see it as doing any harm at all -- and it might even be helpful.

I always use a compression vest for my patients. Compression vests reduce the chance of hematoma. As far as your case its early post-op ,I would refer any questions to your surgeon.

It is far too early after surgery to make any conclusions. You should continue to follow your surgeon's instructions It will take around 3 months to see close to the final results. Good luck.
