what is scar tissue in Post Surgery and Healing After Surgery

hey guys im about 25 days post op and im wondering what scare tissue is exactly?

i can feel a hardish lump on both of my nipples, but they have gotton lease dense and hard as time has passed. Is this what scar tissue is, the lump where they made the incision?
