Actually your condition looks bad but in my opinion is easy to fix.  It looks like all gland and any skilled P.S familiar  with gyno  should  be able to correct your problem

This is called a constricted (or tubular) breast deformity -- it is seen in women as well.

Breast tissue is concentrated to a significant extent just below the areola, thereby pushing it out like a tuber (hence the medical term). 

I have treated similar cases.  It will definitely require an incision around the edge of the areola -- and then an assessment of how much lax skin there is.

Tuberous Breast Deformity Gynecomastia can sometimes be treated without additional skin reduction as demonstrated in that link.  This contour comes in many different degrees.  Options depend on many factors best discussed with your surgeon.  Here is another example of Tubular Gynecomastia of the Male Breast treated without skin reduction.

When the deformity is greater or skin quality worse and excess skin of the male chest is a factor, skin reduction techniques may be necessary.
