Unfortunately you won't be able to do it in one visit, Dr. Fielding requires a consultation before hand and won't schedule surgery till after that (could be a month or longer b/w the two dates). Also when you call his office you won't be speaking with him, it'll be his secretary: Leslie.
There's a lot of good info here about people going to TO from BC now, if you read through the related threads from the past you'll get a good idea of what the process involves. You can check my post history (I'm in TO right now from Vancouver for surgery), 'Manwithnoname' has also posted very thoroughly about the process as well as some other people from the past.
