I remember a guy who used to post on these boards (nukem) who had his surgery with Dr. Metzner who did a gland excision THROUGH THE ARMPIT.  No scar around the nipple.  He looked great and his only complaint was the scar tissue was getting "hard".  Regardless, he was flat.  I wish more plastic surgeons would perfect this technique through the armpit...
