Who determines?  It is certainly not the patient.  On the other hand, some surgeons are not very experienced in gyne surgery -- and may opt for one or the other.  Most of my cases require both gland excision and lipo to achieve the very best sculpted male chest. Best advice for you:  do your homework and find a plastic surgeon near you who has demonstrated interest and expertise in gyne surgery.  If you can get one or more consults, that would be better.  The best chance you have for a good result is with the first (and hopefully only) operation -- so take your time and choose the surgeon with whom you feel most comfortable and confident.  Do not let the fee be your only determining factor.  IF your surgery turns out well, then the fee will be long forgotten and the results of your operation will last a lifetime.

This is a common question and is covered well here by Dr. Jacobs.I agree with every point he made.Less than 1% of the patients I see can be optimally treated by liposuction alone.
