So I had my surgery 7 weeks ago excision and lipo, all seemed fine until a couple of weeks ago when some denting appeared. Denting since then has improved (I think), but creasing has appeared on the right nipple when arm is by side/muscle flexed.

I have researched a good bit on this site and mostly I have seen that it does'nt really go away...  I had the feeling sorry for myself phase already and have came to terms with the fact that I may have to have some kind of revision surgery.

My biggest fear before the surgery was scars but they actually seem to be healing fine!

Had a follow up appointment with my surgeon this week he showed me how much tissue he had removed etc. I expressed my concerns with him about the creasing and asked was there anything that could be done if it remained like this he just told me to go to the gym build my chest (that is now really flat) up and it may well stretch the skin of the nipple out. He also said that massaging it may help smoothen it out. Other than that I just feel like he fobbed me off and told me he didnt need to see me again. No mention of revision etc if it doesnt pan out, nor even the general sense of wanting to know if I was happy with any of the results. To be honest he doesnt seem to be much of a people person. I left feeling like another € in his pocket.

I know I'm still healing too but I'm just looking for advice...

My questions for any doctors out there are....

Have you ever seen improvement of creasing/folding with massage and muscle gain after a couple of months?

Is there any paticular technique/position of arm that I should use while massaging?

Is there any other tips/tricks to tackle the issue?

If no improvment what kind of surgery would be beneficial?

It is still relatively early in the healing game -- would not rush into any revision surgery until at least 6 months has passed.

There are a number of reasons for the crease -- and without further info and a physical evaluation, I cannot offer any recommendations.  However, in general, once there is a crease, usually it is not improved by massage or building chest muscles, although obviously there is no harm in trying.

I would recommend that you seek a second opinion from a gyne expert in your area -- or consider traveling for such expertise.
