I have had my gynecomastia revision surgery 3 weeks ago. In firsf week everything was normal but in week 2 nipple becomes little bit puffy again. My surgeon says its swelling and just keep massaging the area it will go by itself. Is it swelling or residual gland.

It is difficult to say at this point.  Your surgeon knows how much gland was removed and he is the best person to rely on for advice.

At three weeks, you are still in the very early phase of healing -- so it indeed may be swelling only.  However, only time will tell.

Would strongly recommend you wear a firm compression garment, which should help.

And then be patient for at least 3-4 months before beginning to draw any conclusions.

It's normal for the nipples to be a little puffy early in the post-operative period due to swelling. This should improve with time, so by now, you may be seeing a better contour.
