The only thing that takes care of puffy nips/gynecomastia is surgery.  Find a good doctor and make it happen.  You will be able to be normal and move on with  your life.

Until then buy some small or extra small wife beater tank tops and wear them under your tee shirts.  They are tight enough to press down the fatty/breast tissue and allows you to feel great.

If anyone comments on you wearing the white tanks all the time just tell them you feel more comfortable and clean wearing underwear....

Get the surgery.  It's the best thing you'll ever do for yourself.
